Physical Activity Readiness Questionnaire



The British Shuai Jiao Union (BSJU) requires all new participants complete a Physical Activity Readiness Questionnaire to help us and you decide if it is safe for you to participate in Shuai Jiao and to find out about any problems that may prevent you from exercising. Many health benefits are associated with regular exercise and the completion this form is a sensible and necessary first step if you are planning to increase the amount of physical activity that you do. For most people physical activity should not pose any problems or hazards. This form has been designed to identify the minority of people for whom physical activity might not be appropriate, or those who should seek medical advice concerning the type of activity most suitable for them.

Please note that an Access code is required to enable you to submit your questionnaire. The Access Code can only be obtained from the BSJU Secretary. Please contact the BSJU Secretary to obtain your Access Code.

Personal Details

Photograph
Your photograph should be a clear recent colour photograph, that meets UK Passport Quality standards.

The standard size used in photo booths in the UK are acceptable (if you are outside the UK not all photo booths use this standard size), and your photograph should not be trimmed or cut down from a larger photograph to the size of a standard passport photograph.

Health Questions

Please respond to each of the questions below by indicating either 'yes' or 'no' or select your answer from the drop-down menu.

1. Have you been diagnosed with a heart condition and/or been instructed to participate in medically supervised activity?

2. Have you ever felt pain in your chest during physical activity?

3. Have you ever experienced chest pains when not participating in physical activity?

4. Have you suffered with palpitations?

5. Have you suffered with dizziness or fainting?

6. Have you been diagnosed with high blood pressure?

7. Have you recently undergone surgery (within two months)?

8. Do you have existing bone or joint problems?

9. Do you experience shortness of breath?

10. Do you have Asthma?

11. Do you have Haemophilia?

12. Do you have Epilepsy?

13. Do you have Diabetes Mellitus?

14. Do you take prescribed medication that may affect your ability to exercise?

15. Are you pregnant or have you recently given birth (within the last six weeks)?


If you have answered ‘Yes’ to any of the questions listed from 1 to 15, we may require you to obtain written consent form your doctor before agreeing to allow you to participate in Shuai Jiao. We suggest that you talk to your doctor before becoming more physically active. Please tell your doctor about this questionnaire and the questions to which you have answered ‘Yes’.


16. Do you smoke regularly (20 tobacco products a week or more)?


17. Do you drink alcohol regularly (21 units a week or more)?


18. Do you have a cholesterol level that is greater than 4.0 mmol/l?


19. Do you consider yourself to be overweight?


20. Do you take anabolic steroids and/or performance enhancing drugs?


21. Do you have AIDS or are you HIV Positive?


22. Do you have Hepatitis or are you a carrier?


If you proceed with participation in Shuai Jiao and during this period your health changes so that you would subsequently answer ‘Yes’ to any of the questions in this questionnaire it is your responsibility to inform us immediately as you may need to change or possibly suspend your training. If you feel unwell due to a temporary illness (e.g. influenza or COVID-19 symptoms) it is advisable to stop training and recover properly.

If you unsure of the answer to any of the 22 questions listed, you may wish to seek medical advice before completing this form to ensure that it is completed accurately. Failure to complete this form accurately may invalidate your application for membership of the BSJU and its associated insurance scheme.

  • Apply for Membership
    You can apply for membership of the BSJU, if you are a Shuai Jiao practictioner and an exponent of one (or more) of the five principle styles of Shuai Jiao, or if you practice Shuai Jiao as a component of your martial arts system (including wrestling, grappling, and throwing). On successful registration you will then become part of a global family with a history spanning some 4,000 years; and access to national, international, and world-level events.

  • Safeguarding
    The BSJU recognise that everyone has a responsiblity to keep themselves and others safe, and people involved in Shuai Jiao should know what to do if they have any concerns. The BSJU are working to support our members, parents, carers, and volunteers to safeguard children and adults at risk. The BSJU is committed to maintaining safeguarding best practice, reflecting our corporate responsibility to safeguard all participants, effectively.

  • Renew Membership
    You need to renew your BSJU membership annually to remain an active member of the BSJU. It is important that you do not allow your membership to lapse, as you are expected to be insured and licensed, as a requirement of participaion in Shuai Jiao practice. Shuai Jiao, as a martial art, carries with it inherent risks of personal injury. Participants must have adequate insurance in place, which can be obtained through the BSJU membership process.

  • Coaching Standards
    The BSJU recognise that Coaches (Instructors) play a critical role in the development of any sporting activity, and in the lives of the people that they coach. Our Coaches (Instructors) ensure that participation in Shuai Jiao is a positive experience aiming to ensure a high quality and safe approach to training. The BSJU Coach Development Programme (CDP) is delivered online and face-to-face, and designed for ‘teaching’ and ‘performance development’.

  • DBS Check Service
    You can apply for a DBS check as part of the 'martial arts workforce' through our partner organisation: SCiMA Education Limited (SCiMA). Enhanced DBS check are required, as a condition of membership, for all Heads of Association, Coaches (Instructors), Designated Safeguarding Officers (DSOs), and people in a position of trust. The practice of Shuai Jiao will involve physical contact, and it is essential that ALL participants are safeguarded, appropriately.
